Congratulations on your assistantship or fellowship award! If you are new to the University, you are required to complete a number of forms before you begin working (state and federal tax forms, I-9, personal information form). These forms are available in the Human Resources Office in the Salus Center (3545 Lafayette Avenue). However, the forms are available for downloading at the following sites:

You can then bring the completed forms to Human Resources, along with the required documentation for completion of the I-9. These forms must be on file prior to the start of your award.

If you have been employed by the University within the past 12 months, you do not need to complete the above forms unless you need to change your tax status.
